At the risk of increasing the size of an already large work I have endeavoured to retain the fulness and completeness which characterised previous editions. In order to assist in meeting this difficulty of size, and in response to numerous requests from practitioners a portion of this edition is being published on thin paper, thus making the book lighter and easier to handle and, it is hoped, increasing its usefulness as a work of reference.
